Description

Module/Solution Overview

The City4Age intervention module is a web app that facilitates geriatricians and practitioners to organise, control and schedule the treatment (intervention) to their clients monitoring their activities through City4Age Dashboard. In combination with the Intervention Delivery Module the geriatricians and practitioners are able to provide consistent services to the elderly end-users.

An installation of the City4age Intervention consists of several data components: a data base of “resources” (content for intervention), a data base of “templates” (way to organize messages for a resource), a database of “actors” (aged individuals, geriatricians, staff). In terms of SW components the following need to be installed: intervention designer (for planning delivery), Intervention_engines (creating messages and scheduling them), and intervention_delivery (actually sending messages over the various channels).

The Intervention Module allows to create single and a customized intervention (after the creation of a prescription by the geriatrician) or group intervention (to a huge group of care-receivers).

City4Age Intervention Module

Innovation

The City4Age Intervention Module is a powerful intervention organiser for the geriatricians and practitioners that enhances the services to the elderly end-users. The City4Age Intervention Module was created in respect to national and European legislation for the personal data protection.

Business Impact

The practitioners and geriatricians are able to organise and control their intervention to their clients using a powerful tool such as the City4Age intervention module.

Geriatricians, looking at actual behaviours through detection results and comparing them with past trends, can formulate “prescriptions”, i.e. texts that include recommendations on how to design intervention for each care-receiver to solve present issues or just to stimulate. Caregivers take all the list of available resources (pdf, events, playbills, etc.) and matching geriatricians’ prescriptions with personal profile data design the right intervention format, in terms of how many messages to deliver, from which resources and thanks to which flow (templates).

BusinessImpact
